Aortic valve calcium score: does it correlate with mean transaortic gradient?

چکیده
Abstract Introduction Aortic valvular calcium score (AVCS) is useful in patients with aortic stenosis for whom echocardiography was not conclusive grading its severity. Transcatheter valve implantation (TAVI) an established procedure of treatment symptomatic severe AS. The burden calcification has been associated some TAVI related complications (as perivalvular leaks), but at the same time it well accepted that degree needed to ensure stable anchoring prosthesis annulus. Purpose To assess if there a correlation between and mean transvalvular gradient 6 months after – higher AVCS correlated lower transaortic TAVI? Methods We performed single-center, retrospective cohort study including who underwent preoperative standardized contrast enhanced MSCT available. Clinical echocardiographic data were collected previously (pre-TAVI) follow up (6M-FUP). Results A total 187 included, 54% female age 79.4±9.0 years old. Most had tricuspid (95.7%); 5 bicuspidy 3 bioprothesis. Concerning type, 73.3% new generation main used CoreValve Evolut Pro (33.7%). Also, 38,5% balloon pre-dilation before TAVI. pre-TAVI maximum gradients 76.5±23.2 mmHg 48.3±15.5 mmHg, respectively; area 0.75±0.16 cm2. 2851±1524 AU (Agaston Units); 81.2% women AVCS>1300 74.4% men >2000 AU. Comparing 6M-FUP TAVI, average differential 61±22 40±15 mmHg. negative weak found (pearson coefficient −0.181, p=0.02) −0.191, p=0.014). Discussion conclusion significant predictor death, stroke leaks On other hand, high better seating native annulus during deployment. Nevertheless, did strongly Funding Acknowledgement Type funding sources: None.

Resting Aortic Valve Area at Normal Transaortic Flow Rate Reflects True Valve Area in Suspected Low-Gradient Severe Aortic Stenosis.
OBJECTIVES This study sought to assess the diagnostic impact of stress echocardiography (SE) in patients with suspected low-flow, low-gradient aortic stenosis but normal resting transvalvular flow rate. BACKGROUND SE may help to distinguish between true severe aortic stenosis and pseudosevere aortic stenosis in patients with low aortic valve area (AVA) and mean gradient. However, if rest flow...
متن کاملTransaortic valve-in-valve implantation after previous aortic root homograft.
Catheter-based valve implantation techniques are becoming a viable option in various clinical situations to replace difficult redo open heart surgical procedures. This is a report of a first, to our knowledge, successful valve-in-valve (VinV) transcatheter aortic valve implantation (TAVI) into a homograft through the transaortic (TAo) access route using an Edwards SAPIEN valve prosthesis (Edwar...
متن کاملMinimally invasive transaortic mitral valve repair during aortic valve replacement.
Herein, we report the case of a 77-year-old man who presented with congestive heart failure. Echocardiography and cardiac catheterization revealed severe aortic stenosis with severe mitral regurgitation and a left ventricular ejection fraction of 0.20. Because of comorbidities, the patient was considered to be at high risk for double-valve surgery. In order to reduce the operative risk, a minim...
متن کاملHigh transvalvular pressure gradients on intraoperative transesophageal echocardiography after aortic valve replacement: what does it mean?
Intraoperative transesophageal echocardiography is now used routinely during aortic valve replacement, allowing immediate evaluation of replaced or repaired valves. It is well recognised that high transvalvular pressure gradients can be detected immediately after implantation of a prosthetic aortic valve which may be due to multifactorial confounding variables, including functional phenomena, p...
